Thank you for your participation.
We are conducting a survey on the above and would be grateful if all owners would help by completing the questionnaire.
Why are we doing this:
• To establish the life expectancy of SBTs.
• To find out the causes of death of SBTs.
• To find out how many SBTs have had skin lumps.
• To determine the causes of such lumps. How to fill the survey in:
• Please complete the questionnaire if a dog has died since 1st Jan 1995 in your ownership.
• Please fill in a new form for each dog dying in your ownership since that date. Please photocopy or scan the questionnaire into your computer to produce more copies as required.
• Many questions are ‘yes/no’ or similar so delete as appropriate.
• A comments section is provided to provide further information where requested, or which you may volunteer if considered relevant.
• Do not include dogs dying under three months old, the neonatal period.
• Only age in years is required for dogs over twelve months when dying.
• Giving your own or dog’s name etc is optional but contact/identification may help. Omitting these details may be considered if a dog is put to sleep with temperament problems; these dogs should be included for scientific accuracy.Returning completed forms:
• Completed ‘paper’ forms should be handed or posted to Maureen Ward (EMSBTC) or handed to L.P or A.B. if easier to do.
• Returns, from scanned or computer generated copies, may be made by attaching to email to Linda Payton (SCSBTS).Confidentiality: All information supplied will remain confidential. Anyone with concerns (e.g. dogs with temperament problems) may contact me (A.B) directly.
Anyone wishing more information or advice may contact Maureen, Linda or myself.
Thank you for you anticipated co-operation. Please ask those to whom you have sold dogs to participate in this survey and supply forms as necessary.
