Sample Registeration Documents

If you are looking for advice and information on buying a SBT puppy the Breed Council have drawn up a very useful pamphlet.  Please click here
Take a close look at these examples of Kennel Club registration documents - when you are looking for a puppy, ask to see the registration documents for the dam of the litter (possibly the breeders may have a photocopy of the sire's documents, too) and also the puppies' documents if they have been received from the KC.  Study them carefully and make sure they are genuine "Kennel Club" registration documents.

SAMPLE 'CERTIFICATE OF EYE EXAMINATION' GIVING 'UNAFFECTED' RESULTS, AND ASSOCIATED REGISTRATION DOCUMENT WITH EXAMINER's CERTIFICATION

THE ABOVE IS AN EXAMPLE OF A BONA FIDE 'CERTIFICATE OF EYE EXAMINATION' . PLEASE NOTE IN PARTICULAR THE RESULTS WITHIN THE SECTION: 'INHERITED EYE DISEASE STATUS'. IN THIS EXAMPLE THE DOG TESTED 'UNAFFECTED' WHEN EXAMINED FOR PHPV (Persistent Hyperplastic Primary Vitreous) and HC (Hereditary Cataract). IF THE DOG OR BITCH IN QUESTION HAS TESTED 'UNAFFECTED', THEN HIS/HER 'CERTIFICATE OF EYE EXAMINATION' SHOULD BE MARKED IN THE SAME WAY AS THIS EXAMPLE. THE DOG's KC REGISTRATION DOCUMENT SHOULD ALSO BE CERTIFIED BY THE EYE EXAMINER AT THE TIME OF TESTING (SEE EXAMPLE).

IN ADDITION, YOU SHOULD SATISFY YOURSELF THAT THE DETAILS ON THE 'CERTIFICATE OF EYE EXAMINATION' REFLECT THOSE ON THE CORRESPONDING KC REGISTRATION, WITH PARTICULAR REFERENCE TO THE OWNER/BREEDERS NAME & ADDRESS, THE DOG's REGISTERED NAME AND NUMBER AND OTHER RELEVANT DETAILS.
